Currently, we are unable to accept new patients without a referral. If you are an established patient, or referred to us by an existing patient or provider, please fill out the form below to let us know how we can help. Due to high demand, wait times may be longer than usual.

We do not participate with dental insurance, Medicaid, or Medicare. While we are happy to submit claims on your behalf, our office is considered out-of-network. Any reimbursement from your insurance carrier will be sent directly to the patient.
